All you do is turn the highway after the ramp into 4 lanes (for a 2 lane ramp connecting to a 2 lane highway), then back to 3 after a while. If the game doesn't want to make the 4 lane piece the length you desire, connect a 2 lane small road (any non highway road will probably work) to it at the right place, convert the highway to 4 lanes, then delete the 2 lane road.

The way you did the alignments and showed step by step is great. My gripe is with how you used bridges. AFAIK this would more often be done using an overpass/underpass setup.
